This guy's for necro 3rd rank helm...
Spent forever trying to find this guy up near explorer tower(1500,-200 ish) in LOIO got him to spawn finally, killed him and no brain (http://lucy.allakhazam.com/item.html?id=12409). I couldn't find any proof its 100% drop rate, but from reading over the various threads on allakhazam (http://everquest.allakhazam.com/db/quest.html?quest=99) it seems its a rare mob not a rare drop (no one said anything about not getting the brain after killing him).
I could have just been really unlucky, though :)
