Mage Reaver not spawning Neh’Ashiir

We’ve killed the CoM reaver for the Mage Epic piece in hopes of spawning Neh’Ashiir on two separate occasions and after killing the first reaver nothing spawns out of it, no additional reaver no Neh’Ashiir.

From talking with other members this doesn’t seem to be expected behavior and was recommended to submit a bug request.

I see Neh'Ashiir quite often doing pulls in CoM on my bard and killing the reaver afterwards. I don't think he spawns from a specific reaver he can spawn from any. I've seen him spawn from the one on 3rd floor, and the one that spawns at the front of castle all within the last 3 weeks.
