It's 5-11-26 at approx. 12AM. About 30 minutes ago I turned in a pristine scarab carp. and 23 gold to Vacto Molunel to get the breastplate and he gave me a helm. I haven't tried any of the other turn ins, I just happen to have what I needed for the breastplate. I read about a simular bug that looked like had been fixed. this issue appears to be simular if not the same.

Have you tried turning in another 23 gold to see if you'll get the Breastplate now?

If someone turned in scarab carapaces without any gold, it of might MQd the Helm to you when you did your turn in. If this happened, then the Pristine carapace is still on Vacto

It may just be an issue with how MQs are handled with all 3 of them having gold as part of the turn in

No, I didn't try that.. I've already traveled to the Karanas. I would think there would be maybe a reset timer on the turn ins. That way if someone turns something in and if the requirements of the turn in aren't met with in say, idk... 5 to 10 minutes it would reset... so the next person's turn in doesn't get messed up. I suspect if someone did a turn in, it might have been a while. I don't see just a bunch of new people running around in the zone.

Quest inventory doesn't reset, even upon NPC death. Known issue on development plans to fix. More complicated than it seems (as is most things on an emulator where you aren't allowed to alter the client code)

That would make sense, but such is not implemented. Also I don't think it will be because I don't think that's classic. But I don't know that for sure.

One upside of this is that if you mess up your quest turn in somehow, there's plenty of time for you to gather the rest of the components and turn in. For instance, if you were supposed to turn in A+B+C and you turned in A+B+D and got nothing, hurry out for a C and turn it in, and voila. Hope this helps.
