Nope, i lost my monk epic MQ due to someone before me messing up there MQ, so the warrior who handed in the fists got the epic right away before me handing in my book, no refund was granted

They have a pretty strict 0 policy in regards to MQing no matter the situation

I think they'd reimburse the items if a legitimate bug caused the MQ to fail. But if it's due to someone elses error, naw.
