Staff of Elemental Mastery Fire is dropping off male a forsaken revenants.

This item should only drop after the PoH revamp when they also drop the Staff of Elemental Mastery: Earth.

It looks they were introduced in order of water > earth > air > fire.

This item does indeed come from Lord Innoruuk.
Staff of Elemental Mastery: Water -- Phinigel Atropos (Kedge Keep)
Staff of Elemental Mastery: Earth -- Innoruuk (The Plane of Hate)
Staff of Elemental Mastery: Air -- Plane of Air quest
Staff of Elemental Mastery: Fire -- ???

All these staves are the same stat-wise, yet each are for their specific elemental. Mage only. If anyone has any questions on the skyplane quest, Let me know. I'll be glad to fill you in.

That is correct. Kedge was itemized, then PoH released, then PoSky, then the revamp.

always wondered why it wasnt in the mage epic and apparently it was bc the quest came first

I searched for evidence as well and only found mentions of fire staff w the Mistress of Scorn...I dont even think the male revs on Blue should drop it post revamp (they currently do as well).

EQlizers beastiary listing for it Male Rev from Dec 2000 which is welllll into Kunark but before Mistress of Scorn exists

They added Magi Ptsa s earth staff to normal male revs eventually bc of how hard it still was for mages to get that kill and if they added that its likely fire was tossed on there too so it was spicy which one a male rev was holding when you found it...so its likely P99 devs quickly noticed Earth staff shouldnt be on a male rev but were unsure ab fire so just left it
