There is more than one round of upgrades during Velious:

June 2001 was a massive upgrade to NToV, ST, and Kael items.
October 2001 was a massive upgrade to 2-handed weapons on top of prior upgrades.

There are other items that have upgrades sprinkled throughout the timeline.

There are other items newly introduced to drop tables towards the end of Velious (or maybe around the October 2001 2-hander upgrades).
