This sounds like there is a conflict between EQ real, your desktop, and your video card. Have you set them all to 809x600 before? If not set all three (make sure in your Nvidia aid program, not sure what it is called, you change it aswell). Also, with your second monitor fucking up, do you have it set as one screen, AMD has eyefinity, does nvidia have an equivalent? If so disable that aswell

Really oddly i know i told you how i solved my similar problem with lowering my oc but i also had some other res and odd problems i solved by doing some things
if you have steam add eqgame.exe as a non steam game put patchme in the launch options make sure steam is NOT set to run as admin or any compatibility settings make sure eqgame.exe is NOT set to run as admin or any comaptibility settings and try and start and run it through steam seems silly but it solved alot of my problems suprisingly | ||

now i know you said your processor isnt overclocked is the stock rate above 4.1/4.2ghz if so try and UNDERclock your cpu to 4.0 then do .1 increments from there untill like 3.2ghz because honestly this is EXACTLY what was happening to me and it was just a cpu timing issue

right steam version is not titanium im saying use the titanium install exe and load it through your steam it helped some issues i had AFTER the cpu one
