At level 35 the mem blur for monsters not at their spawn becomes random instead of 100%. You can still blur stuff without it having to return to spawn but sometimes it takes multiple attempts.

Usual reasons for feign death failure:

--"Falls to the ground" is the random failure that gives the well-known message.
--Auto attack being "on" will cause feign to fail every time. There is no warning message.
--If something starts a spellcast, you feign, then the spellcast lands after you feign, it will break the feign effect even if you resist the spell. There is no failure message for this either.
--Likewise, if a monster is mid-melee swing when you feign, it is possible for it to hit you after you land, which also breaks feign. There is likewise no failure message when this happens. This one does not happen as often, but it does happen; it's more likely if you feign with a crowd of creatures hitting you.

Note that for the majority of failure scenarios, you don't have any direct warning that feign failed, so you have to pay attention and closely watch what's going on.

If you feign with a damage-over-time effect on your target, the feign will work as long as you stay down, but the damage will cause the monster to re-aggro as soon as you stand back up. You have to wait for the damage spell to fade. This can be annoying with something like disease cloud which has a long duration. A monster's damage over time spell on YOU when you feign won't affect anything (if you already had it in advance--see above about spells landing after you feign) aside from losing health while flopped on the ground. Instant invisibility effects like from hide or pre-nerf circlet no longer provide automatic memblur on P1999 like they used to years ago.
