The proc from luminary 2H sword offsets the -cha on brain and then some, with a long duration on top of that.

BIS range slot item for paladin is CT brain by far
just unequip it for lulls

There's an argument for Orb of the Deep Sea. Not universally BIS in all situations, but if you're doing something where there's no real risk of dying and/or you're not being CHed (say, an easy grind session with a non-cleric healer), maximum AC is the most efficient thing you can have. The FT from CT brain is nice if you don't have an enchanter or bard in the group, but if you do, it's pretty negligible. There are situations and group setups where 45 AC is better.

You can also get the orb for low dkp while the brain will cost you a year's worth.

I think it was viable due to bow proc rate being based on main hand delay but that's getting fixed next patch. So it will have the proc rate of a 44dly weap now if unhasted.

Honest question: is the koi orb (45 AC) the BiS for group content? I notice a huge difference between when I wear it or my thunderstone.

The main consideration for this question is how much AC you have without Koi Orb. AC does get softcapped after a certain amount, so you may get diminishing returns if you already have a bunch of high AC pieces equipped. From my understanding, Warriors get the best softcap returns. The untested number provided by the dev who wrote the AC code is 45%. All other classes get a worse percentage below 45%. The number I saw for knights is 33%, so the 45 AC koi Orb would become a 15 AC Koi Orb if you are above the softcap.

Koi Orb would certainly not be BiS while above the Softcap. Personally I think Cazic Brain would still be better, even when below the softcap. But it would be interesting to see some comparison data.

This test on my 60 Shaman was parsing hit data from a level 40 mob. Going from 100 AC to 150 AC (both of these values are below softcap) reduced my melee damage taken by roughly 12%.

Flowing Thought I from Cazic Brain gives you 600 mana per hour, which is basically 3 celestial cleansings. So for Koi Orb to potentially be better, it would need to save you at least 175 × 12 = 2100 HP per hour. When using the 12% reduction number, that would mean you need to take more than 18000 damage per hour for Koi Orb to even be a consideration for BiS.

There shouldn’t be post cap returns at all in velious until the very end and even then it was very small… I want to say like 12%?

Still, devs have done a tremendous job creating a facsimile of classic. I appreciate it greatly.
