Quest Issue: Qeynos Medal of Merit bugged

The last step in the Medal of Merit quest is currently bugged. It appears that someone turned in the note and Gnasher's head to Captain Tillin because he summons the executioner. However, currently in Qeynos, the executioner and Captain Tillin are in a loop where they continuously go through their text, but nothing happens (ie, the executioner doesn't leave to kill McNeal Jacob).
