Quest Issue: 1 Charge on items with multiple charges?

Passing week did Ivandyr's Hoop quest - when Lynuga spat it out at long last, it had only 1 lifetap charge on it out of the 6; last time I did it it had full 6. A buddy did Soulfire today and said it had only 1 charge too (I thought it has 5?)...

So kinda curious if this is intended, a new feature of some random charge amount, or some bug that came up when the recharging thing was patched recently? Anyone else have this happens?
